By submitting this form, the merchant acknowledges and agrees to following: 1) All information and documents provided are true, accurate, and complete 2) The applicant authorizes Everything Financial Professional Corp. and its assigns to disclose all information and documents obtained by the Representative including credit reports, to other persons or entities (Assignees) involved in commercial loans with daily, weekly, or monthly repayment options, or in the purchase of future receivables, including Merchant Cash Advance transactions. Each Assignee is permitted to use such information and documents for potential transactions. 3) The Representative and each Assignee depend on the accuracy and completeness of information and these documents. 4) The Representative, Assigne, and their respective representatives, successors, and designees are authorized to request and receive any investigative reports credit reports, statements from creditors or financial institutions, verification of information, or any other information deemed necessary by the Recipient for qualifying Applicant for funding. 5) The Applicant waives and releases any claims against Recipients and any providers related to the request, receipt, or release of information. 6) Each owner/off affirms that he or is authorized to submit this form on behalf of the Merchant.
